A previously well 48 year old man self presents to the emergency department. He complains of  a 2 hour history of sudden onset left sided flank pain radiating down to the groin with associated vomiting. He is pale and sweaty and writhing around in pain. He is tachycardic at 110 bpm. The rest of his observations are normal.
