A 51 year old lady presents with 2 days of central abdominal pain with associated vomiting. She passed a bowel motion initially but nil in the last 24 hours. She is not passing flatus. She has past history of a laparotomy and incisional hernia repair.

An 85 year old gentleman is brought to hospital by ambulance from his nursing home. The staff there report he has been complaining of abdominal pain for a number of days, is off his food and today the staff noticed his abdomen is distended.
