A 72 year old man has been brought to ED by ambulance. The paramedics have pre-notified you that the patient is complaining of central crushing chest pain. They are worried that he is very unwell and request a team to be on standby in resus to receive them.
On arrival the patient is extremely short of breath. His observations are as follows;

Sats 88% on 15 L NRM
HR 110 bpm
BP 75/40
Temp 36.8
GCS 13
