Case 3

A 21 year old man is brought to the Emergency Department by ambulance. His motorbike collided with a car at high speed and he was thrown 20 metres. He was received by a Trauma Team. On arrival to ED his observations are as follows;

 

RR 35,

Sats 95% on 15L O2 via a non rebreather

HR 155,

BP 75/40,

GCS  6

 

His airway is secured. Wide bore access is obtained and bloods are sent including group and cross match. eFAST shows a large amount free fluid in the abdomen. Portable chest and pelvic x-rays are non remarkable

 

His BP is now 70/30. He is pale, mottled and cold.
